Go here to request your free kids toothbrush. Offer valid August 22 through December 31, 2010. One toothbrush per child per address. You can have up to 10 children on one log. More than 10 requires a second log. Each child needs to fill out form and complete log in order to qualify. You can fill it out for them after they complete tasks. Allow 10 to 12 weeks for delivery.
